You deserve to feel confident about your health, and that requires having a simple way to get to your medical records.


Who can see your personal information?

Sometimes, as part of your long-term treatment plans, you’ll need to allow someone else or another medical facility to see your medical records, but don’t worry – they won’t be able to access them without your permission.

Protecting your medical information is a top priority to us, so we’ll have you complete and sign an Authorization to Use or Disclose Protected Health Information Form regardless of who will be getting the information. In certain cases, an additional approval from your doctor may be needed.

 

How can you get copies of your records?

If you need copies of your records, complete and sign a Patient Request for Access to Protected Health Information form.

**The Release of Information window is currently CLOSED due to the COVID-19 pandemic. However, you can still request and receive records via one of our other options below.

You will be able to choose one of the following ways to receive your records:

  • MyChart
  • Paper copy
  • Electronic copy
  • CD*
  • USB*
  • E-mail

*Please note that there may be a small fee to copy and mail your records and for portable media like CDs and USBs.

 

More About MyChart

Looking for an easier way to manage your medical records and health care needs all in one place? You can create an online account through our MyChart tool. You can view your health records 24 hours a day from a computer, smartphone, or tablet and do things like:

  • Check your care instructions
  • Look at test and lab results
  • See your medications, immunizations, allergies and health history
  • Email your doctor’s office with non-urgent medical questions
  • Schedule, confirm or cancel your appointments
  • Ask for prescription refills from your doctor

MyChart should be used for keeping track of your medical history, documents, setting up appointments and other non-urgent medical needs. If you have an urgent medical issue, please call your doctor’s office rather than using MyChart.

How can I send my records to an outside doctor?

If you visit a doctor who doesn’t work at Valleywise Health, we’ll take care of sending your medical records to them for free. Just fill out a Patient Request For Access to Protected Health Information Form and include the doctor’s name, mailing address, phone number and fax number.

Insurance, attorney and disability requests

Sometimes your insurance company, attorney or Disability Determination Service (DDS) will need to request your records. In this case, you’ll need to fill out an Authorization to Use or Disclose Protected Health Information form so we can give them the information. We’ll respond to the request within 30 days, but if the information isn’t available yet or stored off site, we are allowed an extra 30 days to respond. We will notify you in writing about any delays.
