'Terry' James Ballentine - Adult nurse practitioner
Adult nurse practitioner (ANP), Terry Ballentine, is an Advanced Practice Nurse, Board Certified in Adult Health. Terry was awarded his undergraduate degree in Nursing from Grand Canyon University in 1987. In 1997, he obtained his master’s in science from the College of Nursing at Arizona State University.
Terry, a District Medical Group (DMG) primary care provider, is available for consultation at Valleywise Health. Terry has spent the majority of his career in chronic disease management in long term care. He began his work in HIV and Transgender Hormone Replacement therapy in 2019 at Southwest Center for HIV/AIDs.
Terry is also involved in social responsibility by volunteering at the Joshua Tree Feeding Program serving the HIV positive population as well as feed those experiencing homelessness through Trinity Episcopal Cathedral.
