Valleywise Community Health Center – Sunnyslope is scheduled to close on October 28, 2020. The services and most of the care team will transition to the new Valleywise Community Health Center – North Phoenix, located at 19th Avenue and Northern Avenue (2025 W. Northern Avenue, Phoenix AZ 85021).
As part of Valleywise Health’s commitment to providing great care for you and your family, we’re excited to be opening Valleywise Community Health Center – North Phoenix on November 2, 2020. This state-of-the-art facility will offer all the same services you already get and more, including a pharmacy, lab, imaging services and integrated behavioral health.
Remember, you can make an appointment with your health care provider by using our telehealth services over video or phone. We’re always here when you need us.
