As part of Valleywise Health’s commitment to providing great care for you and your family, we’re excited to be opening Valleywise Community Health Center – South Phoenix/Laveen on August 17, 2020. This state-of-the-art facility will offer all the same services you already get and more, including a pharmacy, lab, imaging services and integrated behavioral health.
Valleywise Community Health Center – 7th Avenue is scheduled to close on September 19, 2020. The services and most of the staff members will transition to Valleywise Community Health Center – South Central, located at 33 W. Tamarisk St, Phoenix, AZ 85041. Our South Central location will be a family practice clinic and include additional support from family practice residents.
Please know, the same great team and high-quality care will be available to you at all Valleywise Community Health Centers across Maricopa County.
Remember, you can make an appointment with your health care provider wherever you are by using our telehealth services over video or phone. We’re always here when you need us.
