How can you get copies of your records?

If you need copies of your records, complete and sign a Authorization to Use or Disclose Protected Health Information Form.

How can I send my records to an outside doctor?

If you visit a doctor who doesn’t work at Valleywise Health, we’ll take care of sending your medical records to them for free. Just fill out a Patient Request For Access to Protected Health Information Form and include the doctor’s name, mailing address, phone number and fax number.

How can I send records to a third party?

Sometimes your insurance company, attorney or Disability Determination Service (DDS) will need to request your records. In this case, you’ll need to fill out an Authorization to Use or Disclose Protected Health Information form so we can give them the information. We’ll respond to the request within 30 days, but if the information isn’t available yet or stored off site, we are allowed an extra 30 days to respond. We will notify you in writing about any delays.
