Conveniently located at 19th Ave. and Northern Ave., this state-of-the-art facility offers the same great care you’ve come to expect from Valleywise Health, all in a beautiful, brand new location near you.

Visiting Hours and Guidelines

Everyone is welcome to visit, and we have open visiting hours. However, in some special situations our staff may let you know of changes to those hours based on what’s best for our patients. We understand every situation is unique – please talk to our nursing team to come up with some options.

Appointment Checklist

In order to make sure your experience is as smooth as possible, we suggest arriving at least 20 minutes prior to your first appointment and 15 minutes ahead of time for all following appointments. Remember to bring the following items with you:

  • Your current insurance or AHCCCS card.
  • Your insurance co-pay, deductible or payment in full if you are not covered by insurance. We will collect your payment at check in.
  • A valid photo ID. This can be a driver’s license, passport, military ID, employment badge or student ID.
  • If you need to reschedule your appointment, no problem – please just make sure you call us and let us know at least 24 hours ahead of time.

How to Pay

You can always find treatment at an affordable price here, regardless of your ability to pay. If you don’t have insurance, don’t stress – we’ll make sure your health care is affordable using our Sliding Fee Discount Program and other resources.
