At Valleywise Community Health Center – Avondale, you’ll find a variety of specialty and primary care services that will strengthen your mental and physical health and keep you and your loved ones feeling your best. Conveniently located just off of Van Buren St. and Dysart Rd., we’re ready to care for you via an in-person or telehealth visit.

This location is also home to the First Episode Center, a place dedicated to making sure your adolescent and young adult loved ones are able to live happy lives and pursue their dreams –after or while experiencing psychosis. Meet Your Doctor

Tina McKenzie, MD

Tina McKenzie, MD, is a District Medical Group physician at Valleywise Health. She currently practices Family Medicine at Valleywise Community Health Center – Avondale and Glendale. She studied medicine at Loyola Stritch School of Medicine in Chicago. Dr. McKenzie completed her residency at Banner University Medical Center in Phoenix, AZ. She is board-certified by the American Board of Family Medicine.

Appointment Checklist

In order to make sure your experience is as smooth as possible, we suggest arriving at least 20 minutes prior to your first appointment and 15 minutes ahead of time for all following appointments. Remember to bring the following items with you:

  • Your current insurance or AHCCCS card.
  • Your insurance co-pay, deductible or payment in full if you are not covered by insurance. We will collect your payment at check in.
  • A valid photo ID. This can be a driver’s license, passport, military ID, employment badge or student ID.
  • If you need to reschedule your appointment, no problem – please just make sure you call us and let us know at least 24 hours ahead of time.

How to Pay

You can always find treatment at an affordable price here, regardless of your ability to pay. If you don’t have insurance, don’t stress – we’ll make sure your health care is affordable using our Sliding Fee Discount Program and other resources.
