At Valleywise Community Health Center – Avondale, you’ll find a variety of specialty and primary care services that will strengthen your mental and physical health and keep you and your loved ones feeling your best. Conveniently located just off of Van Buren St. and Dysart Rd., we’re ready to care for you via an in-person or telehealth visit.
This location is also home to the First Episode Center, a place dedicated to making sure your adolescent and young adult loved ones are able to live happy lives and pursue their dreams –after or while experiencing psychosis. These services include preventive care, diagnosis and treatment of medical conditions and long-term care.

Everyone is welcome to visit, and we have open visiting hours. However, in some special situations our staff may let you know of changes to those hours based on what’s best for our patients. We understand every situation is unique – please talk to our nursing team to come up with some options.
